Psychiatry for older adults and their families

Time to listen, and room for your family on the call

Crest is two prescribers on Cape Coral Parkway. Visits are long enough to talk, one medicine changes at a time, and a daughter, a son or a friend can join from anywhere by video or phone.

Two chairs, a desk, and a screen for your family

The consulting room is quiet, with the lights low and the door closed. The laptop on the desk is how a family member joins by video.

Care

Four kinds of visit, each with one job

Crest sees adults 60 and older, and the people who help look after them.

  • Your first session

Sixty minutes. Your history, every medicine you take, and what you want to be different. Nothing is prescribed until we have talked.

    About the first session
  • Medication reviews

Thirty minutes, on a schedule. One change at a time, a date to look again, and your pharmacy's list checked against ours.

    About reviews
  • Video visits

From your own chair, anywhere in Florida. A link we send you, large buttons, and a phone call if the internet is down.

    About video visits
  • Family on the call

A son in Ohio, a daughter across town, a friend who drives. They join by video or phone, with your say-so.

Who is in the visit

Alone, with family on the call, or with family in the room

You decide, each time, and you can change your mind in the middle of a visit.

How it works

From first call to first visit

Four steps, and nothing to fill in online first.

  • 01

    Call or write

Call the office or send a message. Leave medical details out; those come in person.

  • 02

    We check your coverage

We look at your Medicare and any plan you have, and tell you what we find before the visit.

  • 03

    Choose who comes

Pick a time on the scheduling page, here or by video, and tell us who, if anyone, should join you.

  • 04

    Come for sixty minutes

Bring every bottle in a bag. You leave with a plan, a date for the next visit, and a large-print summary.

Prescribers

Two prescribers, one policy

Each license is listed at the foot of every page, with the board that issued it.

    Arcelia Soto

    Psychiatric-mental health nurse practitioner

    • Advanced practice registered nurse, psychiatric-mental health nurse practitioner · FL Lic. # 000000

Where we see people

In the office from across Lee County, by video from anywhere in Florida

A video visit needs you to be in Florida when it happens, because that is where we are licensed. If you winter here, that means the months you are here.

Near the office

  • Cape Coral
  • Matlacha
  • Pine Island
  • North Fort Myers
  • Bokeelia

A longer drive, or by video

  • Lehigh Acres
  • Fort Myers Beach
  • Sanibel
  • St. James City
  • Alva

Anywhere else in Florida, we see you by video. Outside Florida we cannot treat you while you are there, but we can see you again when you are back.

Prescribing policy

How we change a medicine

A short policy, the same for every patient and both prescribers.

  • No prescription before an evaluation, and none from a message alone
  • One change at a time, so you and your prescriber can tell what each change did
  • Every change has a date to look again, written in your summary
  • You are told what to watch for, and whom to call, before you leave
  • A medicine is lowered or stopped only with your prescriber, never on your own

How fees work

The questions nearly everyone asks before a first visit, answered plainly. No fee is quoted here; yours is given in writing.

  • Insurance we work with

Crest bills Medicare Part B for first sessions and medication reviews, and for video visits where your plan allows them. We also bill many commercial plans. Medicare Advantage and supplement plans differ, so we check yours before the first visit and tell you what we find.

    • Medicare Part B

    • Most PPO plans

  • If you pay yourself

Without insurance, your fee is set by the length of the visit and given to you in writing before you start. A small number of reduced-fee visits are held each month; ask when you call. HSA and FSA cards work. We ask for 24 hours' notice to change or cancel a visit.

  • Receipts for reimbursement

After a visit you can ask for an itemized receipt (a superbill) with the date, the kind of visit and a diagnosis code, to send to a plan that reimburses care outside its network. Your insurer sees the code, and your plan decides whether it pays anything, so ask it before you start.

  • An estimate in advance

If you pay without insurance, you have the right to a written Good Faith Estimate of what your care will cost, before you start. We send yours before the first visit. If a bill comes to $400 or more above the estimate, you can dispute it. The estimate is not a contract.

  • Checking your benefits

Our front desk checks your Medicare and any plan you have before the first visit, and calls you with what they find, including what you may owe. If something changes, we call before the visit, not after.

Questions

What people ask before a first session

Do you take my insurance?

We bill Medicare Part B and many commercial plans. Medicare Advantage and supplement plans differ, so our front desk checks yours before the first visit and calls you with what they find. If we are not the right fit for your plan, we will say so and suggest where to look.

What happens in the first session?

It lasts 60 minutes. Your prescriber asks about your history, how you have been and every medicine you take now, explains how Crest works and the limits of confidentiality, and you decide the next step together. Nothing is prescribed until you have talked.

Can a family member join my visit?

Yes, in the room, by video or by phone, if you say so. You choose who joins and can ask them to step out at any point. A written permission comes first, and your summary goes to them only if you say so.

Are you licensed where I live?

We are licensed in Florida. A video visit needs you to be in Florida at the time of the visit, which is the months you are here if you spend part of the year elsewhere. Your family member can join from any state.

Do you prescribe controlled medicine by video?

Not on a first video visit. Some medicines for sleep, anxiety and attention are controlled by federal and Florida law, and the rules for video change. We tell you what applies to you before the visit, and we never prescribe a controlled medicine from a message.

What is a Good Faith Estimate?

If you pay without insurance, you have the right to a written estimate of your costs before you start. We send yours before the first visit. If a bill comes to $400 or more above it, you can dispute the bill. The estimate is not a contract.
